Through funding made possible by Saskatchewan Seniors Mechanism, the Town of Shaunavon has entered into an agreement to be a pilot initiative for Social Prescribing for Older Adults. This 3.5 year initiative will include the hiring of a half-time (20 hours/week) contract position to implement social prescribing for older adults in the community.
Job Summary: Social prescribing is a way for medical professionals to connect individuals to a range of non-clinical services in the community to help improve the health and well-being of older adults. It addresses underlying non-medical causes as it promotes integrated community-based support and care and helps to reduce some of the dependence on health service providers. A Community Connector will empower older adults to take control of their health and wellbeing through a holistic approach by connecting this population with community groups and services for practical and emotional support. Interested candidates should submit a cover letter and resume to: spshaunavon@gmail.com
Applications are being accepted until March 31, or until a suitable candidate is found. Anticipated start date: ASAP, preferably in April. This position will report to a Steering Committee of the Town of Shaunavon, responsible for the Social Prescribing Initiative.
Please contact the Steering Committee at spshaunavon@gmail.com if you have questions. A complete job description will be available to candidates who are chosen for an interview, or visit https://www.shaunavon.com/.../community-connector-for.../ for qualifications and details.
